Runt is associated with the Runtfeast wish. This wish only appears in Act 3 if you have not completed Broodfeast in Act 2.

Runtfeast has the same quest objectives as Broodfeast, requiring you to obtain 15 Skewered Organs, 35 Shredded Organs, and 10 Seared Organs. Giving these items to Runt rewards you with the Longclaw.
